Submit Samples to SGS-IPS

To ensure our testing laboratory is able to quickly and correctly process your request, we ask that you please follow the steps below:

  1. Click to download and complete a Test Services Request Form and include a fully-completed copy with your samples.
  2. Make sure your samples are clearly labeled or marked with a sample code and any other pertinent information (e.g. machine direction, side to be tested, etc.)
  3. Any special instructions for testing must be printed and included with your sample submission. Otherwise standard test methods will be used.
  4. Ship the samples and your completed Test Services Request Form to:

  5. If you prefer to drop off a sample, you may do so during our normal working hours of 8:00 am – 4:30 pm Monday to Friday. If you need to drop samples off before or after normal work hours please call Customer Service at +1 (920) 749-3040 ext. 1 to make arrangements.

SGS-IPS reserves the right to refuse to receive or analyze samples that are deemed to be unsafe to handle in our facilities (or for any other reason).

SGS-IPS does not accept radioactive material.

Prior consultation is required before shipment of samples that are biologically active.

Certain samples may require field preservation, cold shipment or other specialized handling to maintain sample integrity. Check with SGS-IPS to verify the sample quantities and/or handling requirements for testing.

A purchase order or credit card is required for all jobs. Prepayment in the amount of 50% of anticipated testing fees may be required before testing will begin on any projects over $5000. International customers will be required to pay in full before testing can begin. Accepted payment methods include checks drawn on US Banks, Visa, Master Card, American Express and wire transfers (fees apply.)
